UFCW247 Benefits Trust Fund – Frequently Asked Questions

After three full calendar months of work.

All employees are eligible for benefits on the first day of the month after their first three full calendar months of work. It doesn’t matter if you are part-time or full-time – you get benefits!

Example: If you were hired on May 15, you must work the rest of May and then the next three full calendar months (June, July, and August). Your benefits would start on September 1.

Sometimes, employers take a while to report hours to the Plan. However, if you were eligible for coverage on a day that you used your benefits, you would still be covered by the plan.

To get your benefits, you must register with the Benefits Administrator (PBAS).

There are a couple of ways to do this:

  • You can register using the Online Benefits Portal (RECOMMENDED): ufcw247.drawbridge.ca; OR
  • You can download a printable version of the Registration Form here. You can then send the completed form to the Benefits Administrator (PBAS) by email, fax, or mail using the contact information below.

Please note these are NOT the same forms you filled out when you were first hired. If you cannot access this form, please ask a Shop Steward for assistance or contact PBAS to have them send you the form and/or assist you with the registration process.

Your benefits end when your employment ends (your last day worked).

Please note that benefits can end or be suspended in some situations:

  • If you are on an approved leave of absence, your benefit coverage continues for three months from the date you started your leave. After that, your coverage ends.
  • If you are on maternity, parental, or adoption leave, your coverage will continue during your leave, but you must complete a Missing Hours Form after 4 weeks to ensure continuation of coverage. For more information, please read the section on “What happens to my benefits if I’m away from work?”

Your death benefit coverage continues until you reach age 65 or your employment ends.

If you are on an approved leave of absence, your coverage continues for three months from the date you started your leave.

Because of the way the Employer reports hours to the Benefits Administrator, we recommend completing a Missing Hours Form if you are away for any period longer than four weeks.

If your approved leave of absence continues beyond three months, your coverage will be suspended until you return to work. That means you will not be covered or reimbursed for any expenses during that time.

There are a few exceptions where your coverage may continue:

  • If you are on maternity leave, you will continue to have benefits for up to 17 weeks from the date your leave begins.
  • If you are on parental or adoption leave, you will continue to have benefits for up to 61 weeks from the date that leave begins.
  • If you combine maternity and parental leaves, you will continue to have benefits for a maximum of 78 weeks from the week of the child’s birth.

 

Make sure you complete a Missing Hours Form for any leave longer than 4 weeks

To ensure your benefits continue when you are on an approved leave of absence or have reduced hours because of a disability, you must complete and submit a Missing Hours Form, which is available here and on the Online Benefits Portal ufcw247.drawbridge.ca.

The Missing Hours Form must be signed by the employer (i.e. store admin) and submitted to the Benefits Administrator (PBAS) by fax (604-945-7657), email (247benefits@pbas.ca), or mail to:

The Death Benefit coverage continues until you reach age 65 or your employment ends.

Yes! Your Benefits Plan has a direct pay card that allows your provider to submit claims electronically on your behalf, without you having to mail in a claim.

Once you register, you should receive an electronic version of your card by email. You can save (keep in your email, download, or take a screenshot) or print this card for your convenience. Please note that no physical cards are provided.

Your Benefits Card is also accessible within the online Benefits Portal (ufcw247.drawbridge.ca).

You should present your Benefits Card to your office (i.e. medical or dental), which should allow them to access the electronic pay-direct system and have your claim processed immediately. Your provider will advise you of any amount owing or not covered, if applicable.

For quicker claims processing, we encourage members to use their Benefits Card at pharmacies, dental offices, and offices of vision, physiotherapy, chiropractor or massage therapy practitioners.

If you end up paying directly for any service, you may submit your claim via:

  1. Online Benefits Portal (drawbridge.ca) or
  2. Manually (by fax, mail, or email) to the Benefits Administrator (PBAS) with the completed claim form and receipts.

You can also request a printed claim form from your employer or PBAS (see their contact below).

When making a claim, please note that receipts must be received by the Benefits Administrator no more than 18 months from the date of purchase, or they will not be accepted.

Once received by mail, fax, or email, claims can take 1-2 business days to be processed. Reimbursement cheques are sent out every two weeks.

The quickest and most convenient way to be reimbursed for benefits claims is to sign up for direct deposit. You can do this through the online Benefits Portal (ufcw247.drawbridge.ca).

You can also set up direct deposit by mail. Just visit mypbas.pbas.ca/ufcw247directdepositform to download a copy of the direct deposit form. Submit the completed form and a void cheque with your next mailed claim form.

If your claim was not covered or was only partially covered, we can look into it.

But first, start by contacting the Benefits Administrator (PBAS) to verify the details of your coverage and the reason why your claim was not covered.

It can be helpful to provide PBAS with the following information:

  • Type of claim (Vision, Drug, Extended Health Care, Dental)
  • How was your claim submitted? (email/fax/mail/direct-pay/ online Benefits Portal)
  • Date your claim was submitted
  • Total amount of the claim

Please note: UFCW Local 247 Union Representatives cannot access your specific benefits information. The UFCW Local 247 union office does not process claims or have access to your benefits records.

It’s easy to use your Benefits Card at the pharmacy.

Setup procedures for pharmacies are available below and here.

For electronic processing of prescriptions, your pharmacy will require the 9-digit Certificate ID number found on the front of the Benefits Card. This is the number that they will enter into the “Client ID” field. MDM does not require a carrier number or a group policy number. However, should your pharmacy’s software require an entry in that field, advise the pharmacist to enter “1000.”

Your pharmacist may need to contact their software provider to confirm which processing code they have assigned to MDM.

Note: Your pharmacist can call the Pharmacy Support telephone number on the back of your Benefits Card (1-800-838-1531) should they require assistance with a claim or for a walk-through of processing your first MDM claim.

If your healthcare provider has not registered with PBAS, you will need to manually submit your Claim Form and receipts to PBAS for reimbursement of eligible claims within 18 months of the expense.

In the online Benefits Portal (ufcw247.drawbridge.ca), you can search for your drug by name or by using the Drug Identification Number (DIN) associated with the medication you’ve been prescribed.

If you do not know the DIN, your pharmacist can provide it to you.

Many dental offices submit claims to your Benefits Plan on your behalf.

All dental benefit payments are based on the procedures in the 2023 BC Dental Association Suggested Fee Guide.

If you or your health or dental provider wish to verify your coverage, please contact the Benefits Administrator at 1-800-663-7977, or email 247benefits@pbas.ca.

Is a pre-determination required?

For any proposed claims over $300, we strongly encourage you to have your dentist submit a pre-authorization, in writing, to the Benefits Administrator (contact info below). They will let you know exactly what is covered. By doing so, you will avoid unnecessary treatment or excessive charges that may not be covered by this plan.

Group B Coverage

Dental benefits are extended to your spouse or eligible dependant(s) only when you have met the eligibility requirements of Group B coverage, which would mean you are either full-time, full-time on modified work hours due to disability, or part-time but working more than 128 hours in each of three consecutive four-week pay periods.

Your dental office can submit claims electronically by adding PBAS to their system. You can share the setup procedures (available for download here) with them before your visit.

Members of the Benefits Plan are in either Group A or Group B, depending on how many hours they work in a consecutive four-week pay period. The main difference is that your dental benefits are extended to your spouse or eligible dependant(s) when you are in Group B.

  • Group A – you worked less than 128 hours in each of three consecutive four-week pay periods
  • Group B – you worked more than 128 hours each of three consecutive four-week pay periods, are a full-time employee (in an actual full-time position), or are a full-time employee on a modified work plan due to disability.

It is important to keep track of your hours. To know how many hours you have worked over the four-week pay period, you can also check with your payroll (i.e. Store Admin) or the Benefits Administrator (PBAS).

Your vision, physiotherapy, chiropractor or massage therapy practitioners can register for the PBAS Health Provider Portal using the following steps:

  1. Visit provider.pbas.ca
  2. Select “Register” and enter email and password.
  3. Complete the registration and submit.

Once approved, your Health Practitioner will be able to submit your claims electronically. This registration is usually confirmed in one business day.
