UFCW UNION PENSION PLAN The following members, participate in the UFCW Union Pension Plan: Extra Foods # 8551, 8553, 8568, 8570, 8572, 8576 DENTAL PLAN INFO FOR CLERK II's & JUNIOR CLERKS (Save-On/Safeway/Overwaitea Members) The Trustees of the UFCW Health and Welfare Trust have approved offering dental plan coverage to eligible Junior Clerks, Clerk II's, Demonstrators and Training Clerks in the Overwaitea and Safeway Divisions. The new coverage negotiated by the Union means that a greater number of Junior Clerks, Demonstrators, and Training Clerks (Save-On/Overwaitea), and Clerk IIs and Production Clerks (Safeway) will qualify for benefits. Your extended health and dental coverage is funded by contributions which the Union negotiated with your Employer You do not make contributions to this Plan. Please get in touch with your Division Administrator (contact information is below) if you have questions regarding your coverage that are not addressed in the Questions and Answers below. How is this benefit coverage paid for? Your extended health and dental coverage is funded by contributions that the Union negotiated with your Employer. You do not make contributions to this Plan. a) You have a minimum of six (6) months continuous employment; b) Your are paid for an average of 22 hours per week over 13 consecutive weeks; c) You are 18 years of age or older. Additionally, the Trustees of the Plan make other considerations from time to time to determine eligibility. The dental coverage will reimburse you (and any eligible dependents) for dental expenses, subject to some maximums, limitations and exclusions as follows: Plan A: Basic Services - Exams, X-Rays, Fillings, Root Canals Plan B: Major Services - Dentures, Crowns, Bridges Plan C: Orthodontic Services - Braces Please refer to the enclosed "Benefit Summary" for further details. Please note that you must have provincial Medical Services Plan (MSP) coverage in effect at the time the expense is incurred in order to be eligible for any reimbursement. Do I need to enroll in order to submit a claim? If you are qualifying for coverage for the first time, you need to complete the enrollment form. Once completed, return it to the Overwaitea Benefits Department (if you are a Save-On/Overwaitea employee) or the Safeway Human Resources Department in Calgary (if you are a Safeway employee).How do I make a claim? Safeway members: Please contact the store or the Safeway Human Resources Department in Calgary for a claim form. Your benefit plan number with Pacific Blue Cross is E039353 and your member ID is your social insurance number. For dental claims, ask your dental office if they will submit the claim electronically to Pacific Blue Cross. Save-On/Overwaitea members: Please pick up a form from the benefit form holder in your store or contact the Overwaitea Langley Office Benefits Department for a claim form. Your contract number with Sun Life Canada is 25121 and your member ID is your Social Insurance Number. Dental claims can often be made electronically through your dental office by simply providing your contract number and social insurance number. When does coverage begin? Coverage will begin for claims incurred on and after January 1, 2008. How can I obtain more information? Save-On/Overwaitea members: Call the Overwaitea Langley Benefits Department - in the Lower Mainland at 604-882-7828 or toll free at 1-877-643-7200. Safeway members: Call the Safeway Human Resources Department at 1-888-310-1318 ext 3381 or you may call Pacific Blue Cross at 604-419-2000 (Lower Mainland) or 1-888-275-4672 (outside Lower Mainland).
